    Return Policy & Reseller Policy

    OCuSOFT® Return Goods Policy

    Authorization:  Prior to returning products, a Return Goods Authorization (RGA) Form must be obtained by contacting OCuSOFT Inc. Customer Service Department at (800) 233-5469. PLEASE, have the following information available when calling:

    1.          Customer Name
    2.          Invoice Number
    3.          Date of Invoice
    4.          Lot Number of Product(s) being returned
    5.          Expiration Date of Product(s) being returned
    6.          Reason for Return

    OCuSOFT® will issue credit for products returned to our facility if they meet the following criteria:

    1.          Products must be in original packaging, unopened and in same condition as when shipped
    2.          Return must include a copy of invoice or be accompanied by an invoice date and number
    3.          Return must be within 30 days of invoice
    4.          Product(s) must be listed on Return Goods Authorization Form

    Items that are NOT eligible for credit:

    •            Product with less than 6 months expiration dating
    •            Product requiring refrigeration
    •            Product purchased on Monthly Specials or other offer featuring short dated items
    •            Product not listed on the Return Goods Authorization Form
    •            Freight charges and call tags

     

    All returns are subject to final count and acceptance by OCuSOFT Inc. Credit will be issued only for those products that meet the above criteria.

     

     

    INSTRUCTIONS FOR RETURNING PRODUCT:

    1.          Call OCuSOFT® Customer Service at (800) 233-5469 to obtain a Return Goods Authorization Form.
    2.          Complete and sign the form. Referring to the form, write the RGA number on the OCuSOFT® address label.
    3.          Pack the items securely in a box or the package used to deliver the item.
    4.          Place the completed, signed form in the box with the items being returned.
    5.          Return to:

                6.         Drop the package off at your designated carrier’s location to be shipped ground. Send all returns PREPAID. We

                            recommend shipping the package using a carrier that provides a tracking number and insurance. OCuSOFT® is

                            not responsible for loss or damage during shipment and cannot issue credit for product not received.
